These Terms & Conditions (“Terms”) govern your access to and use of the BAM HEALTH website located at https://bamhealth.org (the “Website”), including all tele-health services, patient portals, memberships, and related services offered through BAM HEALTH (“BAM HEALTH,” “we,” “our,” or “us”).

BAM HEALTH provides access to tele-health services through licensed healthcare providers.
Telehealth allows patients to receive medical consultations using secure electronic communications. While tele-health offers convenient access to healthcare, it is not appropriate for every medical condition and is not intended to replace comprehensive in-person medical care when necessary.
Our providers determine, in their sole professional judgment, whether tele-health is appropriate for your specific condition.
Our services are not intended for medical emergencies.

Medical services available through this platform are provided exclusively by independently licensed healthcare professionals who are authorized to practice medicine in the state where the patient is physically located during the consultation.

Virtual Primary Care Membership Disclaimer
The BAM HEALTH Virtual Primary Care & Annual Lab Membership is a direct tele-health membership and is not health insurance. Membership fees do not cover DEA Schedule II controlled substances, narcotics, sedatives, or stimulants, medical emergencies (call 911 or visit the nearest emergency room for urgent/life-threatening symptoms), in-person procedures, complex disability/DOT/FAA physical forms, formal specialist referrals, outside diagnostic imaging, or the retail cost of prescription medications at the pharmacy. The included Quest "Starter General Health" lab panel is drawn at a local Quest Diagnostics location; optional add-on diagnostic labs are billed at wholesale member rates, and optional in-home mobile phlebotomy blood draws are subject to an additional convenience fee.
